Description
Provide professional nutrition assessment, dietary counselling and individualized nutrition care plans for clients with chronic diseases, nutritional risks and other health conditions.
Deliver nutrition assessment, counselling and follow-up interventions for clients enrolled in the Chronic Disease Co-Care Pilot Scheme (CDCC) and referred from Nurse Clinics and primary healthcare services.
Collaborate with nurses, doctors and allied health professionals in multidisciplinary case management and care planning.
Requirements
Holder of a Postgraduate Diploma or Bachelor Degree in Dietetics, Nutrition or related discipline, with at least 3 years of post-qualification relevant working experience.
Registered Dietitian with valid professional credentials.
